Office Policies

Appointments

To schedule an appointment please call or fill out the Appointment Request Form:

New Patients are always welcomed.

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. 

Everyone at our office is at your service. Please check out our online forms to see if you can save yourself some time and have them ready for when you come into the office.


Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice.  This courtesy, on your part, will make it possible to give your appointment to another Patient.


Billing / Payment 

All billing inquiries are to be directed to our office (click here to contact our offices). Our staff is prepared to answer any questions you may have regarding payments.

Payment is due at the time of service for all non-participating insurance companies.  Co-payment is also due at the time of service. Click here to view all the insurance plans we accept.

Patients are responsible for notifying our office of any changes in their home billing address/telephone numbers and with their primary/secondary insurance coverage.  Forms of payment accepted by the office are cash, personal checks, money orders and the following credit cards:

 


Emergencies

Locust Grove Family Medicine/Reliance Family Care is open throughout the day Monday to Friday from 9.00 AM to 5.00 PM and 9:00AM to 1:00PM on Saturday. The switchboard is open from 8.15 am to 6pm but closes for lunch hour between 1.00 PM and 2.00 PM. When our clinic is closed during lunch hour, an answering system gives details of how to contact a doctor should urgent medical attention be required. Click here to contact our offices now.
